Health Assessment (PHA)
Personal Health Assessment (PHA) provides a valuable snapshot of your overall health. By participating year after year, you can track your progress towards better health! It's fast and easy!

**Please note employees are required to take the Personal Health Assessment (PHA) annually to continue having the Wellness Program benefits such as the free gym access and reduced Weight Watchers fees. Attendance is required for those who want to continue or enroll in the Wellness Program for the October 1, 2016- September 30, 2017 plan year. To clarify, even if you attended an PHA in the Summer/Fall of 2015, you must attend again this year.
For employees with Florida Health Care Plans and Florida Blue who have covered dependents over 18 only the employee needs to participate in the health assessment for your covered dependents to also receive the gym access.
