Freedom Elementary Clinic is open daily from
7:30am until 2:30pm. Please see the resources below for clinic and health

As this year is coming to an end, if your student has any medication at school, please prepare to pick it up no later than June 4th by 2PM. If it is not picked up, we are required to dispose of them according to Volusia County School Board administration of medication guidelines.
The forms that you will need to prepare for the 2021-2022 school year should have come home with your student and are available on the school website under the clinic tab also. The medication order forms will need to be filled out by a doctor after July 1st, 2021. We require one form per medication needed for your student. Please make sure it is filled out entirely. The medication orders must match the medication label EXACTLY.
If it is an emergency medication, please use the emergency care plan form to provide us with the details and information regarding the medication in addition to the medication order form.

are an Asthma Friendly School!
The American Lung Association is offering
parents the Free Asthma Basics training. Parents can create a login and
take the one hour asthma course to learn ways to assist their child. Once
complete, there is a certificate provided.
