Health Care Reform

Proof of Health Insurance Coverage for the 2015 Tax Year

This is the first year all employees will be required to provide Form 1095-C as required under the Health Care Reform law. This form will verify your health coverage under the Volusia County Schools group insurance. Form 1095-C will be mailed to employees through the US Mail no later than January 31, 2016 to the address shown in our records. To ensure you receive your Form 1095-C, please log on to the ePortal and update/confirm your correct mailing address.

Update (January 4, 2016): Employeess will use the information on these forms to verify (if ever required) coverage for each month during the year. Employees do not file these forms with their tax return. They should be placed in records with other important tax documents.

  • Should employees wait to file until they receive these forms?
It is not necessary to wait for Forms 1095-B or Form 1095-C in order to file. Some taxpayers may not receive a Form 1095-B or Form 1095-C by the time they are ready to file their 2015 tax return. While the information on these forms may assist in preparing a return (Yes or No answer), they are not required. Individual taxpayers will generally not be affected by this extension and should file their returns as they normally would. Like last year, taxpayers can prepare and file their returns using other information about their health insurance. You should not attach any of these forms to your tax return.
